Hijacking packages for fun and profit

When is it acceptable to take over somebody else's package? How long should we let inactive "maintainers" block progress? Is it ever OK to be rude and hijack? There have been some heated discussions about this on the mailing lists recently. Can we work out a better policy in real life?
